To make that sort of weapon, you'll need a high blacksmithing skill, as well as a good sagecrafting skill. So to answer your question, yes it is worth it, with the right build. To answer your chakram question, no you cannot.
If I'm remembering correctly Chakrams only take Utility Gems, so the only element they can have is one of the 3 basic Fire, Ice, Lighting elements. However you may be lucky enough to stumble upon a Chakram that comes with poison or bleeding damage. You just can't make one. It comes with Lightning, Piercing, and Bleeding damage, so I love them.
